Elevate Fitness cancellations
We understand that circumstances may change, and our members may need to cancel their gym membership at the end of their contracts. To ensure a smooth cancellation process, we have established the following cancellation policy:
Those who wish to cancel their membership must complete the cancellation form below at least one month prior to the contract end date. This allows us to process the cancellation and update our records accordingly.
If a cancellation form is not submitted by the required deadline, the membership will automatically renew for another term, and the member will be responsible for all associated fees.
Please note that cancellation forms must be completed below, and cannot be submitted in person or via phone or email.
